Workshop Participant Informed Consent:
I, the participant, willingly and voluntarily agree to participate in the Workshop hosted by Wild and Free Painting, LLC. I understand and acknowledge the following:
Assumption of Risks: I am aware that participation in creative workshops involves certain risks, including but not limited to physical injury, emotional distress, or damage to personal property. I willingly assume all such risks.
Release of Liability: I hereby release and discharge [Your Business Name], its organizers, instructors, and any associated personnel from any and all claims, liabilities, demands, or actions for personal injury, property damage, or any other loss arising out of or in connection with my participation in the workshop.
Photography and Recording Release: I grant Wild and Free Painting, LLC permission to use photographs and videos taken during the workshop for promotional and marketing purposes, without any compensation to me. Wild and Free Painting, LLC will ask permission to take a photograph or video of me first.
Code of Conduct: I agree to abide by the workshop's code of conduct and respect the rights and well-being of other participants, instructors, and staff.
Process Painting Participant Informed Consent:
Thank you for choosing to participate in a 6 Week Process Painting Series. I am honored to journey alongside you in this experience.
I, the participant, willingly and voluntarily agree to participate in a Process Painting Series hosted by Wild and Free Painting, LLC.
I understand and acknowledge the following:
Process Painting is a technique designed to work on areas of concern or dissatisfaction in your life as well as develop growth, insight, and awareness of patterns relating to self and others. It can help you connect more deeply to your intuition and improve your overall well-being. Previous participants share they have experienced deeper more meaningful relationships, as well as, improved self-image, mood, and sense of community. It is important to understand that healing and growth is difficult and at times some discomfort may occur which I am trained to help you process and work through.
Confidentiality: Our groups are not the same as therapy and are not intended to replace psychiatric care, psychotherapy or medical treatment. Because they are not considered therapy, these groups do not fall under HIPAA rules and regulations. Confidentiality during groups will be encouraged but cannot be guaranteed. Please share what you feel comfortable sharing.
Cancellation/Insurance/Missed Groups: Payment is due at registration. Should you need to cancel your participation after your pre-paid package purchase you will receive a full refund minus a $25 processing fee in addition to any services that have already been rendered. Insurance will not cover this service since these groups do not subscribe to the medical model, which requires a diagnosis and CPT Code. I do my best to provide make-up groups. If you missed a group in your series, you are welcome to join a make-up group at any time. Should you need to miss two groups please reach out to discuss.
